These versatile instruments could be inserted into the mouth!!! rectum!!! or vagina!!! of the accused and then expanded in the cavity causing pain (duh) and irrevocable internal damage.
The orifice on the receiving end of the device was symbolic of the offense- heretics and liars got the oral pear, passive male homosexuals got the anal pear and women who were considered "witches" or committed such atrocities as having a miscarriage got the vaginal pear.
Added Bonus: Some pears featured sharp prongs for EXTRA throat and cervix mutilation:

For some reason, a lot of care went into beautifying The Pear of Anguish with intricate embellishments. Well, I guess if I'm going to get a horribly cruel metal object shoved into one of my cavities, it would be nice if it was a work of art!!!!!

The things people create never cease to amaze me.
Imagination is a very important element in torture i believe. Perhaps this tool was ornated in order to strengthen it's impact on the psyche.
Medieval torture devices can be pleasurable to fantasize about (the pain they may have caused, the situations they were used in, etc.) to some, it's a fairly popular entertainment ( see also "the beauty of death we all adore" ), partly because they're considered to be requsites of a dark, overcome era. So there are two things one might wonder: 1.were these instruments really effective in a way a tool should be? 2. Is the human purpose, the intention, or the delicate execution of these antique torture devices really things of the past?

I don't want you to get the wrong impression: i'm not into torture at all. Neither antique, nor contemporary.
